Address Update Form

For those with problems using HTML forms, and AOL, use the Manual Address Update Form.
Note: Please update your app as well, 1 per serson, as it is the important one; this is so we don't loose you in mid move.

You can get an application form for updating all your information. There are two versions to try to cover the print problems- Ver 1 and Ver. 2 (It needs 1 to 1.5 inch left margin, legible, some right margin, all on one page). They may or may not printout reasonably, depending on your setup (browser, printer, fonts, etc.) so first use the print preview. Because of the legal and print problems, hand holding by a member who can help may be needed. Try a test print first to see what works best. I get the hard copy. It is a good idea to keep a copy, and review it for updates (medical, doctor, job, insurance, etc).
